    Annual Reports Our Annual Reports provide a comprehensive overview of our organization's accomplishments, progress, and impact each year; they are a testament to our ongoing commitment to improving the care of patients and working with partners toward improving health in our community. From April 1, 2024 to March 31, 2025, the Division Find a Family Doctor Program attached 9,843 patients to a family doctor. 14 new physicians came to the area, for a total of 348 members. 8 new clinics opened their doors in the Central Okanagan. The launch of the Clinic Grant Program supported 22 clinics and 87 providers with nearly $70,000 in funding, empowering clinics to lead projects in MOA development, planetary hea lth and cultural safety. The Central Okanagan Division of Family Practice (CODFP) is proud to offer our community family practice clinics and provider groups a one-time financial grant to support your ideas to improve your Patient Medical Home practices and to build your Primary Care Networks. Our objective is to encourage as many clinics as possible to access the grants for a project that will make a meaningful difference to your clinic, staff and patients. To be clear: grants are intended for short projects on one of this year's topics. For more information on what grants cannot be used for, see the list of exclusions and FAQ below. ELIGIBILITY REQUIREMENTS — Who is eligible to receive a grant? The clinic & project grants are available to family physicians working in the Central Okanagan. Specifically: The physician practices in the Central Okanagan and is not 100% virtual. The clinic is primarily MSP-funded, and has at least one practicing, primarily MSP-funded Family Physician. Clinics that are NOT eligible: Clinics without Family Physicians. Clinics owned and operated by Interior Health (or another Health Authority). For further questions about applying as a group of physicians or partnering with other clinics, see the FAQ section at the bottom of this page. EXCLUSIONS — What can grant funding NOT be used for? Grant funding cannot be used towards: Clinical training or purchasing clinical equipment used by physicians and/or clinicians that involve direct or indirect patient care, or patient information/data. Physician time related to patient care. Clinical trials Projects which are supported by other sources of funding (e.g. DoBC, FPSC etc.) This includes: Staff training on new technology (e.g. online booking system, EMR, new software/ equipment, or online security). FUNDING CALCULATIONS — What is your clinic's maximum grant amount? Maximum grant amounts are funded based on the number of family physician* full-time equivalent (FTE). If a physician works one day per week, that is 0.2 FTE and five days per week is 1.0 FTE. Clinics with: 0.1 to 1.9 family physician FTE will receive $2,000 2.0 – 4.9 family physician FTEs will receive $4,500 5.0 or more family physician FTEs will receive $7.500 For example, if your clinic has 2 full-time family physicians and 3 family physicians that see patients half-time, the clinic is eligible for $4,500 (2 + 1.5 FTEs = 3.5 FTEs). * Funding is based on Family Physicians working in the clinic (not specialists, NPs, allied health, etc.) While the maximum grant funding is determined by your clinic’s total FTE, the actual grant amount awarded will be determined based on receipts submitted in your end of project report. FUNDING TIMELINE — When will the grant money be awarded? A clinic (or group) will receive the initial 80% of the funds shortly after project approval. A clinic (or group) will receive the remaining 20% of project funds upon project completion. We will send you an end of project template to provide a brief summary of your project, after which you will receive the remaining funding. How to do a Project Ready to get started? Follow the timeline below: 1. Plan | 2. Apply | 3. Run | 4. Report 1 Plan Your Project Estimated Time: 20–30 min The initial planning of your project should take no more than 30 minutes. Simply choose a project option from one of this year's project topics , and ensure that you are within the program parameters . Got questions? Note that at least one family physician must sign off on the application. We are committed to ensuring that your projects are approved. Once you have successfully applied and been approved, you will receive the first 80% of funding. 3 Run Your Project Estimated Time: Project Dependant What exactly this looks like, and how long it takes, depends on the details of your project—but all projects must be completed by March 15th, 2026. If you wish to sustain the project past this point, all ongoing costs are your responsibility, and you must still complete an "end-of-project" report. 4 Complete an End-of-Project Report by March 15 Estimated Time: 30 min END-OF-PROJECT REPORTS ARE DUE BY: March 15th, 2026 An End-of-Project Report template will be sent to you in February (or earlier by request), and must be completed to receive the remaining 20% of funding. For each topic, we have described three "featured" project options for you to choose from, but you can also propose your own idea (as long as it stays on-topic). Bring in a facilitator to support advanced access scheduling design. Option B: Triage & Workflow Optimization A clinic grant will support optimizing and creating workflows: Train MOAs and nurses in standardized triage scripts to ensure patients are booked into the most appropriate appointment type. Develop and test call scripts, intake processes, and workflows to prioritize same-day needs. Share learnings across clinics through peer-to-peer sessions. Option C: Sustaining Same-Day Access A clinic grant will support clinics to: Suppor regular "huddles" or review sessions where staff assess daily demand and adjust processes. Provide staff release time to analyze data (missed calls, demand vs. supply, wait times). Use quality improvement methods (Plan-Do-Study-Act cycles) to test and refine changes. Option D: Propose Your Own Project Use your imagination and plan your own great same-day access project. Make sure your project falls within our funding criteria. Grant funding cannot be used for: Clinical training or purchasing clinical equipment used by physicians and/or clinicians that involve direct or indirect patient care, or information/data (e.g. exam room tables, PPE, etc.) Physician time related to patient care Projects which are supported by other sources of funding (e.g. DoBC, FPSC, etc.), this includes staff training on new technology (e.g. online booking system, EMR, new software/equipment, or online security) and purchase of hardware or software Retreats, catering, social events, or appreciation gifts. You could do this by creating informational handouts for your patients on environmental-related health topics (e.g. switching from pMDI's to DPI's, proper drug disposal, or PaRx Park Prescriptions, chemicals or pesticides, etc), or community events Option B: Create an internal policy or planning document around sustainable prescribing practices, sustainable procurement, or a general clinic sustainability plan. This can include creating a manual outlining how to develop more sustainable prescribing practices, writing a policy on how your clinic can reduce its environmental impact by changing its purchasing practices, or creating a general sustainability plan that outlines your clinic's goals for reducing its environmental impacts. Option C: Capital Improvements For example: LED lighting, motion sensitive switches, HVAC, bike racks, low-flow toilets and faucets, window tint, implementing composting (Valley Pro Recycling) , self-closing doors, investing in green cleaning products and reusable exam table covers. Research and implement possible improvement actions such as those listed above that reduce your clinic's environmental impact or makes it more energy efficient. Team-based care integration: Skills for collaboration with multidisciplinary teams, including allied health professionals and physicians. A clinic grant would assist with: Developing or purchasing training modules tailored to clinic-specific needs. Stipends or salary support for MOAs to attend training during work hours. Hiring a professional trainer or consultant to lead workshops. Strategies for reducing errors in documentation and communication. Providing compensation for time spent attending educational sessions. Organizing clinic-specific training. Option D: Propose Your Own Project Use your imagination and plan your own great MOA development project. Make sure your project falls within our funding criteria. Frequently Asked Questions Why these topics? The topics for the 2025-26 Clinic and Project Grant Program were selected based on feedback from our members over the past year. They reflect areas that are important to our community, including planetary health & clinic sustainability, supporting MOA development, cultural safety in care, and enhancing same-day patient access. We understand that other areas, such as maternity, attachment, recruitment, or projects under active Shared Care initiatives, are also high priority. These types of projects are funded through other existing sources or programs dedicated to those specific needs. Can the grants be used towards the purchase of computers or other IT related software/hardware? No. Clinic grants cannot be used towards the purchase of computer hardware or IT related software. Will the grant cover ongoing costs associated with the projects? No. The grant will cover the clinic’s initial project costs, and it is the clinic’s responsibility to pay for any ongoing costs beyond the initial project. Can multiple clinics partner on a project? Yes. Clinics may work together on a joint project that positively benefits each clinic. Each clinic will be awarded the grant based on the number of physicians working at each clinic, up to $7,500 per clinic. Only one person needs to submit the grant request on behalf of all clinics involved. Individual physicians from separate clinics can also apply together as a group on a project. Note that each individual physician can only participate in one grant application. Can multiple physicians in a clinic submit different grant applications? The application is for the entire clinic or group of family physicians named in the application. A single application can include multiple ideas that all support the improvement of Patient Medical Homes and building Primary Care Networks. Each clinic or group may request one grant for a project of its choice. This means a physician can only participate in one grant application per year. We therefore encourage clinic owners and clinic managers representing an individual clinic (or representing a group of clinics) to apply on behalf of the entire clinic.
