The Central Okanagan Division of Family Practice (CODFP) is proud to offer our community family practice clinics and provider groups a one-time financial grant to support your ideas to improve your Patient Medical Home practices and to build your Primary Care Networks. Our objective is to encourage as many clinics as possible to access the grants for a project that will make a meaningful difference to your clinic, staff and patients. To be clear: grants are intended for short projects on one of this year's topics or you can also propose your own topic. For more information on what grants cannot be used for, see the list of exclusions below. ELIGIBILITY REQUIREMENTS — Who is eligible to receive a grant? The clinic & project grants are available to family physicians working in the Central Okanagan. Specifically: The physician practices in the Central Okanagan and is not 100% virtual. The clinic is primarily MSP-funded, and has at least one practicing, primarily MSP-funded Family Physician. Clinics that are NOT eligible: Clinics without Family Physicians. Clinics owned and operated by Interior Health (or another Health Authority). For further questions about applying as a group of physicians or partnering with other clinics, see the FAQ section at the bottom of this page. EXCLUSIONS — What can grant funding NOT be used for? Grant funding cannot be used towards: Clinical training or purchasing clinical equipment used by physicians and/or clinicians that involve direct or indirect patient care, or patient information/data. Medical research. Physician time related to patient care. Projects which are supported by other sources of funding (e.g. DoBC, FPSC, etc,). This includes staff training on new technology (e.g. online booking system, EMR, new software/equipment, or online security). Purchase of software or hardware. Retreats, catering, social events, or appreciation gifts. Clinical equipment and tools. Office renovations or permanent fixtures, including counters, cabinetry, flooring, or other structural improvements. For more questions, please see the FAQ section at the bottom of this page. FUNDING CALCULATIONS — What is your clinic's maximum grant amount? Maximum grant amounts are funded based on the number of family physician* full-time equivalent (FTE). If a physician works one day per week, that is 0.2 FTE and five days per week is 1.0 FTE. Clinics with: 0.1 to 1.9 family physician FTE will receive $2,000 2.0 – 4.9 family physician FTEs will receive $4,500 5.0 or more family physician FTEs will receive $7.500 For example, if your clinic has 2 full-time family physicians and 3 family physicians that see patients half-time, the clinic is eligible for $4,500 (2 + 1.5 FTEs = 3.5 FTEs). * Funding is based on Family Physicians working in the clinic (not specialists, NPs, allied health, etc.) While the maximum grant funding is determined by your clinic’s total FTE, the actual grant amount awarded will be determined based on receipts submitted in your end of project questionnaire. FUNDING TIMELINE — When will the grant money be awarded? A clinic (or group) will receive the initial 80% of the funds shortly after project approval. A clinic (or group) will receive the remaining 20% of project funds upon project completion. We will send you an end of project questionnaire to provide a brief summary of your project, after which you will receive the remaining funding. How to do a Project Ready to get started? Follow the timeline below: 1 PLAN Plan Your Project Estimated Time: 20-30 min The initial planning of your project should take no more than 30 minutes. Simply choose a project option from one of this year's project topics , and ensure that you are within the program parameters . Got questions? If you wish to sustain the project past this point, all ongoing costs are your responsibility, and you must still complete an "end-of-project" report. 2 APPLY Submit Your Application Estimated Time: 15-30 min APPLICATIONS ARE DUE BY: December 31st, 2025 In order to complete the application, all you will need to provide is the family physicians involved, FP hours worked in your clinic, project details, and expected outcomes. Note that at least one family physician must sign off on the application. We are committed to ensuring that your projects are approved. Once you have successfully applied and been approved, you will receive the first 80% of funding. The topics for the 2026-2027 Clinic and Project Grant Program were selected based on feedback from our members over the past year. They reflect areas that are important to our community, including planetary health & clinic sustainability, supporting MOA development, cultural safety in care, and enhancing clinic workflows and/or patient access. We recognize other topics are important to you, and we will always accept applications for topics outside of the ones listed, as long as the project fits into the parameters of our funding agreements. Will the grant cover ongoing costs associated with the projects? No. The grant will cover the clinic’s initial project costs, and it is the clinic’s responsibility to pay for any ongoing costs beyond the initial project. If my project does not fit within the scope of clinic grants, where can I find alternative means of funding? FPSC offers funding for clinics to perform panel management and to implement Health Data Coalition’s EMR data portal. Can multiple clinics partner on a project? Yes. Multiple Clinics or physicians from separate clinics may work together on a joint project that positively benefits all involved. Grant amounts will be awarded based on the number of physicians involved in the project, up to $7,500 per project.Only one person should submit the grant request on behalf of all clinics or physicians involved. Can multiple physicians in a clinic submit different grant applications? Yes. Multiple physicians from the same clinic can submit different grant applications, provided each physician participates in only one application per year. Applications can be submitted on behalf of an individual physician or a group of physicians within a clinic. If a group is applying together, one physician should serve as the lead applicant on behalf of the group. Parameters & Details 01 PURPOSE — What is the purpose of the grant? The Central Okanagan Division of Family Practice (CODFP) is proud to offer our community family practice clinics and provider groups a one-time financial grant to support your ideas to improve your Patient Medical Home practices and to build your Primary Care Networks. Our objective is to encourage as many clinics as possible to access the grants for a project that will make a meaningful difference to your clinic, staff and patients. To be clear: grants are intended for short projects on one of this year's topics or you can also propose your own topic. For more information on what grants cannot be used for, see the list of exclusions below. 02 ELIGIBILITY REQUIREMENTS — Who is eligible to receive a grant? The clinic & project grants are available to family physicians working in the Central Okanagan. Specifically: The physician practices in the Central Okanagan and is not 100% virtual. The clinic is primarily MSP-funded, and has at least one practicing, primarily MSP-funded Family Physician. Clinics that are NOT eligible: Clinics without Family Physicians. Clinics owned and operated by Interior Health (or another Health Authority). For further questions about applying as a group of physicians or partnering with other clinics, see the FAQ section at the bottom of this page. 03 EXCLUSIONS — What can grant funding NOT be used for? Grant funding cannot be used towards: Clinical training or purchasing clinical equipment used by physicians and/or clinicians that involve direct or indirect patient care, or patient information/data. Medical research. Physician time related to patient care. Projects which are supported by other sources of funding (e.g. DoBC, FPSC, etc,). This includes staff training on new technology (e.g. online booking system, EMR, new software/equipment, or online security). Purchase of software or hardware. Retreats, catering, social events, or appreciation gifts. Clinical equipment and tools. Office renovations or permanent fixtures, including counters, cabinetry, flooring, or other structural improvements. For more questions, please see the FAQ section at the bottom of this page. 04 FUNDING CALCULATIONS — What is your clinic's maximum grant amount? Maximum grant amounts are funded based on the number of family physician* full-time equivalent (FTE). If a physician works one day per week, that is 0.2 FTE and five days per week is 1.0 FTE. Clinics with: 0.1 to 1.9 family physician FTE will receive $2,000 2.0 – 4.9 family physician FTEs will receive $4,500 5.0 or more family physician FTEs will receive $7.500 For example, if your clinic has 2 full-time family physicians and 3 family physicians that see patients half-time, the clinic is eligible for $4,500 (2 + 1.5 FTEs = 3.5 FTEs). * Funding is based on Family Physicians working in the clinic (not specialists, NPs, allied health, etc.) While the maximum grant funding is determined by your clinic’s total FTE, the actual grant amount awarded will be determined based on receipts submitted in your end of project questionnaire. 05 FUNDING TIMELINE — When will the grant money be awarded? A clinic (or group) will receive the initial 80% of the funds shortly after project approval. A clinic (or group) will receive the remaining 20% of project funds upon project completion. We will send you an end of project questionnaire to provide a brief summary of your project, after which you will receive the remaining funding. How to do a Project Ready to get started? Follow the timeline below: 1 PLAN Plan Your Project Estimated Time: 20-30 min The initial planning of your project should take no more than 30 minutes. Simply choose a project option from one of this year's project topics , and ensure that you are within the program parameters . Got questions? If you wish to sustain the project past this point, all ongoing costs are your responsibility, and you must still complete an "end-of-project" questionnaire. 2 APPLY Submit Your Application Estimated Time: 15-30 min APPLICATIONS ARE DUE BY: December 1st, 2026 In order to complete the application, all you will need to provide is the family physicians involved, FP hours worked in your clinic, project details, and expected outcomes. Note that at least one family physician must sign off on the application. We are committed to ensuring that your projects are approved. Once you have successfully applied and been approved, you will receive the first 80% of funding. The topics for the 2026-2027 Clinic and Project Grant Program were selected based on feedback from our members over the past year. They reflect areas that are important to our community, including planetary health & clinic sustainability, supporting MOA development, cultural safety in care, and enhancing clinic workflows and/or patient access. We recognize other topics are important to you, and we will always accept applications for topics outside of the ones listed, as long as the project fits into the parameters of our funding agreements. If you have questions or are not sure if your project aligns with one of the topics or our funding agreements, please reach out to us at members@codivision.ca. 02 Will the grant cover ongoing costs associated with the projects? No. The grant will cover the clinic’s initial project costs, and it is the clinic’s responsibility to pay for any ongoing costs beyond the initial project. 03 If my project does not fit within the scope of clinic grants, where can I find alternative means of funding? FPSC offers funding for clinics to perform panel management and to implement Health Data Coalition’s EMR data portal. Yes. Multiple Clinics or physicians from separate clinics may work together on a joint project that positively benefits all involved. Grant amounts will be awarded based on the number of physicians involved in the project, up to $7,500 per project.Only one person should submit the grant request on behalf of all clinics or physicians involved. 05 Can multiple physicians in a clinic submit different grant applications? Yes. Multiple physicians from the same clinic can submit different grant applications, provided each physician participates in only one application per year. Applications can be submitted on behalf of an individual physician or a group of physicians within a clinic. If a group is applying together, one physician should serve as the lead applicant on behalf of the group. Patient Attachment & Panel Support The Central Okanagan Division helps family physician members attach patients to their panels and provides ongoing support as panels change throughout different stages of practice. Patient Attachment The Division connects patients with a family physician or nurse practitioner through the Health Connect Registry (HCR) —a centralized, equitable system that replaces the internal waitlist and provides a consistent pathway for unattached patients to be matched with local family physicians and nurse practitioners. Through collaboration with community clinics and health partners, and the Provincial Attachment System (PAS), we help prioritize patients with more urgent needs, in line with provincial attachment guidelines. This ensures a fair, consistent approach for patients, family physicians, and nurse practitioners alike. For physicians with space in their practice: Let us Know Your Capacity Panel Support We help family physicians and nurse practitioners manage panel changes during retirement, relocation, practice transitions, or leaves of absence. By planning proactively, we help ensure continuity of care for patients and a smoother process for family physicians and nurse practitioners. Our team offers guidance and resources for navigating panel changes and closures, supporting family physicians and nurse practitioners on leave, and managing ongoing panel capacity. We also collaborate with HealthLink BC and other partners to improve the attachment process and expand family physician and nurse practitioner capacity across the region.
