Join us for an engaging session focused on connecting patients with accessible mental health and wellness supports across the continuum of care.
Learn how Foundry Kelowna, PCN Psychology, and PCN Social Work can support individuals and families through mental health services, counselling, substance use support, primary care, social supports, and connections to community resources.
This session will highlight the unique roles of each service, how they complement primary care, and how healthcare providers can connect patients with the most appropriate supports at the right time.
