Funding Opportunity: Improve Primary Care Access

A new funding opportunity is available to support team-based primary care organizations in improving patient access to care through the Primary Care Access Improvement (PCAI) initiative.
Deadline for applications: September 9, 2026.
The PCAI initiative supports primary care teams in implementing the Advanced Access model, an evidence-informed approach that helps balance appointment availability with patient demand. By improving how teams plan, schedule, and deliver care, Advanced Access can help patients receive timely, appropriate care while maintaining continuity with their care team.
Participating teams may receive:
Up to $15,000 in seed funding
Expert coaching to support implementation and sustainability
Evidence-informed tools and resources
Learning and networking opportunities with peers and colleagues
The Advanced Access model is built on five key pillars that support improved access to care:
Proactively planning for patient needs, team capacity, and predictable changes
Regularly monitoring and adjusting the balance between appointment supply and patient demand
Reducing unnecessary variation in appointment and administrative workflows
Strengthening team-based care to ensure patients receive the right care from the right provider at the right time
Fostering clear communication among patients, providers, and care teams
