Could the C-MRP Model Support You and Your Long-Term Care Patients?

If you’re finding it challenging to consistently meet the five LTC best practice expectations and have long-term care patients at Three Links Manor, Lakeview Lodge or Vineyards Residence, the C-MRP model may be an option for you. This option is being shared for your awareness, should it be helpful in supporting your practice and patient care responsibilities.
The five LTC best practice expectations are:
Proactive visits
Meaningful medication reviews
24/7 availability
Completed documentation
Attendance at care conferences
The Collaborative MRP (C-MRP) Model is currently implemented at all three homes, with groups of physicians sharing coverage for residents. This team-based approach provides consistent care while supporting communication and collaboration with care home staff.
