The CODFP is excited to offer the Clinic & Project Grant Program to our community family practice clinics. Clinics are encouraged to access these grants to implement projects that make meaningful differences to clinic operations, staff support and patient experiences, and build your networks.

About the Program
The Clinic & Project Grants Program offers one-time financial grants to family practice clinics to support a short quality improvement project that will make a meaningful difference to your clinic, staff and patients.
At a glance
💸 Funding Amounts

Clinics can receive up to $7,500 in funding based on FTE (Full-Time Equivalents).

80% awarded upon approval.

20% awarded upon completion.

Making it Happen: Your Project Timeline

Estimated Time: 30 min
1. PLAN Your Project
The initial planning of your project should take no more than 30 minutes. Simply choose a project option from one of this year's [4 project topics], and check that is within this year's [project parameters]. Additional project planning support is available by contacting [members@codivision.ca].

Grant parameters at a glance:

Clinics can receive up to $7,500 in funding based on FTE. See FAQ below for more.

80% of funding on approval, 20% of funding upon completing your clinic’s final report and sharing the results.

Grants may be used for clinic staff and/or external vendors to deliver the project.

Grants cannot be used for the purchase of computer hardware, clinical equipment for direct patient care, or where other funding already exists. See FAQ for more.

Grants are for one-time funding, not for ongoing / sustaining costs.

The application must be submitted by a family physician.

Deadline Alert: Apply by December 31, 2025, and complete your project by March 15, 2026
