RPs specialize in managing an average of 25 patients per home, ensuring consistent, quality care, while family physicians can continue caring for their transitioning patients if preferred.   • Important changes to the CLFP Payment

    The Community Longitudinal Family Physician (CLFP) Payment compensates family physicians for the size and complexity of their patient panels. Claims for the 2024 annual payment will open in January, with payments issued in March, while claims for the 2025 payment will open at the beginning of the year, with payments distributed quarterly. Eligible physicians must participate in the Provincial Attachment System (PAS) and submit separate claim forms for 2024 and 2025 to receive both payments.
